Informed Consent Form for Research Involving Human Subjects
Protocol Title: Life Stress, Particulate Matter Exposure, and Inflammatory Reactivity

Approximately, 400 students will be enrolling in this study at UTEP.
You are being asked to participate in the study because you have shown interest to participate, and because you are young and in good health, do not smoke, are not pregnant, understand English, and are currently living in the U.S.
If you decide to enroll in this phase of the study, your involvement will last about 35 minutes spent online filing up a registration form and answering a stress questionnaire.
3. What is involved in the study?
In the event that you become overly distressed please stop your participation. You can contact us (email@example.com 915-747-8324) to set up a consultation with a licensed mental health practitioner to determine your health status and connect you with the appropriate health care services or you can also obtain help directly at the University’s Student Health Center. There is also a risk of loss of confidentiality.
5. What will happen if I am injured in this study?
By answering all the screening questionnaires you will be automatically entered into the sweepstakes of a new Apple iPad Pro. After 400 participants have been screened one participant will be randomly selected to receive the iPad Pro. Also, the results from this screening will also help us determine if you are eligible to participate in the remaining part of this study where you could get a free health screening and up to $100 in incentives.
11. What if I want to withdraw, or am asked to withdraw from this study?
Every effort will be made to keep your information confidential. Your personal information may be disclosed if required by law. Organizations that may inspect and/or copy your research records for quality assurance and data analysis include but are not necessarily limited to UTEP Institutional Review Board. Because of the need to release information to these parties, absolute confidentiality cannot be guaranteed.
